Its a 2012? I would take it back to the dealer and make them fix it! Even if it could be out of warranty, which I doubt, I would make them fix it. Please rate my answer so I get points for it.
What would cause a code 4354 on a 2016 Freightliner Cascadia?
the code means something but only if the engine make , type and transmission make and type is known Freightliners run 3 different engine and up to 4 different transmissions and the code will be specific to that combination take it to a freightliner service center and have it fixed

CHS 71 identifies your chassis control module. Cab 33 indicates your SAM module. The fault codes are going to be on the subsequent screen. Those code numbers are going to tell the technician or anyone what circuit has a problem within the chassis control module.
The first character indicates the system or area of the vehicle where the problem is located. For example, “P” stands for powertrain, “C” for chassis, “B” for body, and “U” for network communication. The second character, usually a “0” or “1,” signifies whether the issue is generic (0) or manufacturer-specific (1).

Locate the fuse box and open the cover.Find the ECM fuse on the fuse diagram.Carefully remove the ECM fuse and wait for 10-15 minutes.Reinsert the ECM fuse into its original location.
ECU reset refers to the process of clearing or resetting the learned adaptive values and settings in the engine control unit. This process is also known as ECM (Engine Control Module) reset or PCM (Powertrain Control Module) reset.
Resetting the PCM puts your car back to its factory settings. Experts usually recommend resetting the PCM to clear a code, after a part replacement, and to solve ignition issues. Next, we see SPN which stands for suspect parameter number, this is used to identify the specific component or condition that is triggering a fault. In our example, we see SPN 3216, when looked up this SPN shows that there is an issue with the Aftertreatment Intake NOx sensor.
The two most common causes of the P0601 error code are that the ECM (or ECU) is failing or has failed or that the ECM/ECU is receiving low voltage. The P0400 error code is defined as an Exhaut Gas Recirculation Flow Malfunction.
Diagnostic Trouble Code (DTCs) -- Trouble codes are how OBDII identifies and communicates to technicians where and what on-board problems exist. The first number in the DTC indicates whether the code is an SAE generic code (applies to all OBDII systems) or is specific to the vehicle manufacturer.
The diagnostic port is behind the driver's seat. With the cable length limited by the engine data bus specifications, that limits where the PCG can mount.
Check your fuses to make sure they haven't gone bad before proceeding to further diagnostics. A clogged port can also be the cause of your OBD scanner not working or reading a code, so make sure to clear out any dust from your OBD port before trying again.
